• How do I become a client at Life Beyond Barriers Rehabilitation Group?

    Starting therapy requires a physician’s prescription. The prescription must be signed and dated by the referring physician. It also needs to include a diagnosis and discipline. (Physical Therapy, Occupational Therapy, Speech Therapy, Wheelchair/Mobility/Equipment Evaluation)

  • What should I bring to my first visit?

    Plan on arriving 15 minutes before your appointment. This allows us to complete any required paperwork. You should also bring:

    1. Insurance card
    2. Photo ID
    3. List of current medications
    4. Any other information that’s been requested

    To provide optimal care from start to finish, our billing specialists will verify your insurance benefits with you prior to your initial evaluation.

    We don’t treat people like a diagnosis. We treat the person, always putting their dreams and goals ahead of systems and protocol.
