Starting therapy at Life Beyond Barriers Rehabilitation Group requires a physician’s prescription. The prescription must be signed and dated by the referring physician. It also needs to include a diagnosis, discipline (Physical Therapy, Occupational Therapy, Speech Therapy, etc.) ordered, and an ICD 10 code for each discipline ordered. For quick, confidential service, please fax the prescription to the scheduling office.
